Certification Instructions
ALL boxes must be checked for your application to be accepted.
Legal Owner Certification - Check the box to certify that you are the legal owner of the property. Ownership is verified through the County Assessor's Website. Only the legal property owner is allowed to apply and sign this application. If you recently purchased your home, you may be denied until you can prove that you are the legal owner. If the property is part of a trust, you will be required to provide documentation that you are part of the trust.
One Retrofit Per Residence - Check the box if you can verify that no previous retrofit has occurred on the property. There is a limit of one retrofit per household per program requirements. If it is determined at a later date that a previous retrofit has occurred and a second retrofit was completed, you will be responsible for paying the costs of the previous retrofit back to the county. If you are not sure, please contact is to verify.
Step 1 - Contractor Quote - Check the box to verify that you will commit to being at home during your scheduled initial inspection to sign the program liability form. Only the legal property owner or a legal spouse can sign the form. This may require that you miss work or other scheduled appointments. You will be responsible for scheduling your initial site inspection. You will have 30 days to complete Step 1.
Step 2a - Fireplace Inspection and Cleaning - After reviewing your quote(s), you must decide if you would like to continue to move forward to Step 2. You will have 30 days to complete Step 2. If you decide to move forward, you must schedule and pay for at your own cost a fireplace inspection and cleaning(if required) by a licensed inspector. You will have 30 days to complete Step 2. You will be required to upload your fireplace inspection/cleaning receipt. Do not complete this step until you have received a program email stating so.
Step 2b - Gas Line Certification - After reviewing your quote(s), you may be required to complete a gas line pressure test by a licensed contractor. You will have 30 days to complete Step 2. If you decide to move forward, you must schedule and pay for at your own cost a gas line pressure test by a licensed contractor. You will have 30 days to complete Step 2. You will be required to upload your gas pressure test receipt stating that your gas lines pass inspection. Do not complete this step until you have received a program email stating so.
Step 3a - Retrofit Installation - Check the box to verify that you will commit to being at home during your scheduled installation to sign the project completion form. Only the legal property owner or a legal spouse can sign the form. This may require that you miss work or other scheduled appointments. You will be responsible for scheduling your retrofit installation. You have 30 days to complete this step. Do not complete this step until you have received a program email stating so.
Step 3b - Post Installation City Permit Inspection - After your gas log set is installed by the contractor, you must be present for the final city permit inspection. Normally the contractor will schedule this for you. However, they will not be there for the inspection itself. This may require that you miss work or other scheduled appointments.
Step 3c - Southwest Gas Meter Tie-in - After your gas log set is installed by the contractor, you must contact Southwest Gas to make the final gas connection. This may require that you miss work or other scheduled appointments.
Overall Project Management - If you feel that you cannot meet the above requirements, you may seek the assistance of family or friends. However, the legal property owner is still required to be on-site to sign all required forms and to understand how to operate the gas log set.
Failure to comply with the program terms and conditions may result in legal action.
